I am not covered
- Call your insurance company to verify that DUROLANE is not a covered benefit.
- Determine if the decision to exclude coverage on your plan was made by your employer, or your insurance company.
- If the decision to exclude coverage was made by your employer, explain to your HR manager that your physician has prescribed you this osteoarthritis treatment. Request that they contact your insurance company to include DUROLANE on your plan on an exception basis.
- If the decision to exclude coverage was made by your insurance company, you can appeal the exclusion by having your physician write a letter stating why you need the treatment. If denied, inform your HR manager. Contact your insurance company’s ombudsman to file a complaint and request a reversal of decision.
